CPG Book forms

You need 2 forms when putting a CPG booklet together. The Awards form, and the Chairman's Planning Guide form. Fill out the boxes on this page (or chose options from the pull-down menus), and hit "Send". You will be sent (via the email addr you enter) the CPG ready to print (via Adobe Acrobat).
